The problem is real, and the intestines can rupture from the pressure without the NG tube going, and since I also have had a couple of real mechanical obstructions requiring emergency surgery, these pseudo obstructions cannot be ignored.
I have read many posts by cancer warriors living without their small intestine for over two years and doing quite well. Why would the surgeon tell us my husband has 1 cm tumors all over his small intestine so he can't remove a portion or the whole thing because you can't live without your small intestine.
Yes you can live without intestines. However it does mean either the rest of your life on TPN which comes with a lot of fairly significant risks (liver failure, line infections, blood clots and kidney failure just to name a few) or transplant (stomach, liver, small bowel, large bowel, and pancreas or some combination there of depending on the patients needs) which comes with its own huge set ...
You're right that people can live for years on TPN without intestines, as long as they don't develop liver problems from long term TPN. You might investigate palliative care, rather than hospice. I don't think that their rules are as tough as hospice, but they're dedicated to relieving pain in those with possibly terminal illness.

My concern is that an ostomy allows air to pass into the small intestine and therefore may kill off the the bacteria that require an oxygen free environment, so we could end up without a lot of necessary bacteria.
